In a serious emergency, call 911 or go to the nearest hospital/trauma center! Follow-up care is to be arranged with one of the district’s designated medical providers. Please contact Human Resources (Beth Collins at 303-245-5936 or beth.collins@bvsd.org) for the list of designated medical providers.

If the injury is not a serious emergency, you must contact Human Resources (Beth Collins at 303-245-5936 or beth.collins@bvsd.org) to make arrangements to see one of the district’s designated medical providers listed below. You are not authorized to see your personal physician and have Workers’ Compensation pay the claim. Human Resources will assist you with your appointment. The Employee Report of Injury/Incident (see the links below for forms) must be filed through Human Resources in order for your bills to be eligible to be paid under Workers’ Compensation.
